Jacob Anthony Forehand, age 14, of Baxley died Friday, May 30, 2014 at Memorial Medical Center in Savannah.
Jacob was born December 1, 1999 in Coffee County. He was a member of First of Assembly of God and served in 1 Way Kid's Ministry. Jacob was an Honor Student at the Appling County Middle School.
Survivors include his parents, Tony & Rebecca Forehand of Baxley; A sister, Courtney Forehand of Baxley; Maternal Grandparents, Rex & Vera Moses & Paternal Grandparents, Curtis & Janet Forehand; Paternal great grandparents, Thelma Herndon & Betty Forehand. Several aunts, uncles & cousins also survive.
Funeral services will be held Friday, June 6, 2014 at 3:00 P.M. from the First Assembly of God with Rev. Ronnie Rentz and Rev. Bryan Gill officiating.
Interment will follow in Omega Cemetery.
Active pallbearers are Caleb Fowler, Lee Fowler, Dan Brannon, Richard Taylor, Doug Jackson & Stephen Smith.
Honorary pallbearers are up & coming 8th grade class of Appling County Middle School
Visitation will be one hour prior to funeral service.
